Biopsy on gastritis tissue. neg for pyoly h. no ulcers. it is not caused by pain meds. and i'm assuming no cancer. what other causes are there?

Gastritis: The first question is whether the biopsy truly shows gastritis. Beyond h. pylori and medications, allergic, autoimmune and other idiopathic causes can be seen. A good pathologist and a good biopsy can often point the direction to the right kind of further investigation. Get a second opinion on the biopsy if you are still symptomatic.
