Ear infection: Ear infections are notorious for being very painful, so yes, they could cause head pain. But if pain persists despite antibiotics, then bring him back for evaluation. Don't ignore possible mastoiditis, sinus infection, meningitis, etc as possible causes of head pain.

Headache & otitis: If your son is already on antibiotics for his ear infection, give it a couple of days to start working, give tylenol (acetaminophen) or ibuprofen for the pain. But if he is not on any treatment he should be seen by his pediatrician . I hope he feel better soon.
