See clinician asap: Your symptoms combined with a history of smoking suggests you may have bronchitis. Asthma or COPD exacerbation, or Pneumonia are less likely but also possible causes. Regardless, you need to see a clinician asap to examine you, possibly perform some diagnostic tests, and determine appropriate treatment. You may not need an antibiotic, but will likely need albuterol inhaler and possibly steroids.
