Gross hematuria: Gross hematuria is visible blood in urine while microhematuria is blood in urine only found upon testing. Painful bloody urine can be caused by kidney stones & infections (bladder & kidney). Painless blood urine is worrisome for bladder cancer, especially in smokers.
