A bulge : The clear sign of a recurent hernia is a bulge or protrusion in the same area as the previous repair. While that might be associated with some pain, pain alone is more likely post surgical at four months. If you are worried go back and see the surgeon who did the previous repair.

Similar to pre-op: A return of the same symptoms you had before the operation could be a sign of rwcurrence, or that the symptoms were not due to the hernia. Presence of a bulge may also be a recurrence, but more likely represents a fluid collection known as a seroma and/or hematoma which will usually resolve over 6 months. See your surgeon to find out.
