How bad is multiple myeloma with 40% abnormal plasma cells in bone marrow?

The number of plasma cells is not the only determinant of prognosis. Are there bone lesions on x-ray? Is kidney function normal? Has there been a response to therapy? Many patients with 40% plasma cells at diagnosis can do well for years.
