Cough: Could be cough generated in back of throat by a post-nasal drip. if you have a tickling sensation at back of throat just a second before you cough that pretty much clinches the diagnosis. Other possibities are habit cough, asthma (even though you have no wheezing), gastroesophageal reflux. You should speak to your MD about this and he's at a loss find another who isn't. Good luck!

Sinusitis?: With the length of time for the cough and congestion, it could be just a bad respiratory virus . but these usually are getting better after 7-10 days. time to be rechecked to make sure the chest isn't involved after this period of time. also to be checked for a possible sinus infection

Recheck: And likely CXR. Any other symptoms? Is there a family history of asthma or other chest problems? Has he used inhalers in the past? Is he immunized? You are right, a 3 week cough needs more assessment. All the best.
