I have a old scar from surgery of appendix and the scar is red, itching and hurting. what could it be?

Sounds like a keloid: but it could be delayed infection. Have a dermatologist look at it. Or your original surgeon, if he/she is still available. I doubt that it is skin cancer, but this is also a possibility however unlikely.
