See Specialist: Given the severity of the symptoms you describe I would recommend seeing a Dermatologist for further workup and evaluation. Difficult to diagnose without seeing lesions in person.

Get seen now: If this is acne, then it is an exacerbation of nodulocystic acne or acne conglobata. It should never have been allowed to get to this stage and it will almost certainly require a systemic emergency Rx followed by isotretinoin. If it is instead a systemic bacterial infection, emergency antibiotic Rx is in order; there are other immune entities such as SAPHO to rule in/out.
