my right nipple itches often daily, not the areola . no dryness, cracks,fever,rash,discharge,discoloration. what could be the cause, should i worry?

No, if GYN exam neg.: You need to see your GYN for A breast exam and evaluation. Early diagnosis of Paget's disease is extremely important.
