GFR: The GFR by MDRD is 10cc/min. Now MDRD may not have been validated for all population it is accurate for most. Preemptive transplant living donor is the best option if possible. Timing is a judgement call and has to be made in person. For most patients renal replacement is indicated once the GFR gets to 10cc. Have a open discussion with your transplant team . Best of luck
