Contributing: Probably can cause some restrictive lung problems eventually but your problem is probably due to your underlying lung disease primarily. Your posture can also eventually lead to circulatory problems and neuro symptoms in your arms (thoracic outlet syndrome). I suspect that your underlying lung problems are severe. I wonder about a alpha-1=antitrypsin deficiency leading to severe emphysema.

Lungs and spine: Not likely. If you had scoliosis that progressed from age 2-4 until now, it is possible. That is when lungs continue to develop along with chest growth. If you had thoracic lordosis (rare), then the thickness of your chest and overall lung capacity would be diminished. Muscle spasm is most likely cause of breathing probs from pain. Luckily, reversible.
