GERD: While the causes for RUQ pain includes many gastric conditions, more common things like GERD or Gastric Reflex Disease need to be ruled out first. Gastric paresis, slowing of gastric motility following an infection etc may be another. The pain in the neck may be a referred pain from a nerve in your diaphragm, near your stomach. If your symptoms persist despite OTC meds for a few days, get checked.

Peptic ulcer disease: Other possibilities too. If pain has been this severe or is moderate,but, present for > 1 week: go see a physician to get a proper exam and work up.
