See the doctor: there are many things that can cause halitosis, and since this appears to be a change in the way his breath usually smells, and because of the whiteness that you see in the back of the throat, he should see his doctor to be evaluated. Although these are not likely to be tonsil stones, they do need to be evaluated. Good luck.
