GYN exam: I would suggest a GYN exam and hormone levels checked including an HCG qualitative and quantitative exam. If you are a virgin or have not had any sexual intercourse, pregnancy is unlikely. You will need a pelvic and medical evaluation.

Not necessarily: Not necessarily, hormonal fluctuations with periods can also cause nausea. To be sure, pick up a home pregnancy test - they are inexpensive and very accurate.

Bleeding in pregnanc: It's possible to be pregnant and still have what appears to be a period..Which is why some people do not realise they are pregnant. There are many reasons for this. The commonest are endometrial sloughing in early pregnancy at a time close to when a period would have occurred , a lower level of hormone called Progesterone which helps stabalize the uterine lining, infection, and bleeding after sex.

IUD: If you have an iud, while it is possible for you to be pregnant, it isn't very likely. If you were, bleeding could happen. If you are worried about this, do a pregnancy test. If it's positive, you need to see your dr asap; all that said, it's very likely not going to be positive.

Possibly: If i understand your question the answer is 'yes, ' you might be pregnant, it's unpredictable following the missed period. You don't give any information on menstrual history, which might allow a more informative answer. You should do a home pregnancy test as soon as convenient.
