Not necessarily: Large studies have shown that gallstones that don't cause symptoms don't require operation. Most people don't, in fact. However, once you start having symptoms due to gallstones such as pain, nausea, vomiting or jaundice, those symptoms tend to worsen over time until gallbladder removal is necessary.
