Anxiety: i would ask your primary care provider to provide you a low dose ativan (lorazepam) 0.5mg tablet prior to your evaluation to calm you down. Just go with a friend, as ativan (lorazepam) can be sedating and impair your driving when trying to go back home from the appointment.

Breathe deeply: I assume you mean ear wax extraction. The best thing is to take some deep breaths, stay calm and try to remember that you'll feel better after the procedure. If you are worried about your anxiety effecting the treatment, then discuss your concerns with your doctor beforehand.
