Tender, spasm: My guess is that the zit causes local tenderness & even muscle spasm in the muscles of the back of the neck & shoulder in sensitive persons. Be sure the zit itself is limited in size & depth, to the superficial skin only, & improving. If not, then do get checked. Hope that helps.
