Can herniated disc cause symptom just sometimes on straight leg test? sometimes i get muscle twitching in feet other times not,same with big toe pain.

Can be intermittent: If a herniated disc causes signs like straight leg raising or sciatic nerve tenderness, it can be intermittent. So can any neurological or muscular sign or symptom. Left alone most resolve with conservative treatment. And some people with herniated disc on imaging studies have no pain.
