Spectrum of Options: Ibuprofen or other nsaids are the best first line option to treat painful cycles with fibroids. If the fibroids are large and may be degenerating, pain may be more severe. Surgery to remove the fibroids, especially davinci robotic myomectomy, give the least discomfort for the longest relief, although the fibroids can return. Hysterectomy can be an option if fertility is not desired.

Uterine embolization: Another good option is uterine artery embolization, a method of cutting off the blood supply to the fibroids causing them to shrink and die. While it does not remove the fibroids it is 95% successful for bleeding and 85% for bulk symptoms. It is performed as an outpatient procedure without the need for stitches and is well tolerated. Performed by interventional radiologists (image guided surgeons).
