Go to ER if severe: U didn't specify upper or lower, but consider addressing signs/symptoms so as to localize the organ involved: biliary, stomach, duodenum, pancreas, biliary tree, colon, small bowel, kidney, skin, muscle. Bleeding, fever, nausea, vomiting, altered stool pattern, blood with stool, tenderness, distension deserve urgent attention. Appendicitis & cholecystitis can cause pain as severe as you describe.

Exam needed: Persistent right sided lower abdominal discomfort,especially with associated tenderness may be due to appendicitis or ovarian pathology and warrants a prompt examination by a physician who will order any appropriate tests in order to make a diagnosis. Right upper abdominal discomfort is seen in gall bladder disease.
