See a doctor: You may need to be x rayed, as the big heavy glass may cause fracture of the bone. Also you have blood collected underneath your nail and it will throb for quite sometime. Doctors may poke a hole and get the blood out, which is a very simple procedure and get the pain away quite quick. Hope that it gets better quick.

X-ray: Would recommend an X-ray of the foot, the intensity of the pain suggests fracture.

Let the blood out.: If you ca sterilize a small needle, you will appreciate relief if you puncture the bubble at it' base. Be prepared to apply pressure with a hopefully sterile gauze pad. If the toe feels a whole lot better, then you may not of fractured your toe. If the pain is just as severe, then you will need an x-ray to confirm fracture. It is not that easy to fracture the great toe, especially in a 23 y.o.
