Constipation: Bleeding from backside (as when you wipe your anus w/toilet paper) is most likely due to fissure or hemorrhoid, especially in 19yo. Without blood mucous stool, fever or weight loss, it's doubtful that bleeding is due to inflammatory bowel disease. Colon cancer is very rare in teenagers. However, combination of constipation & inability to lose weight makes me think of hypothyroidism so check TSH.
