Why do i have nightmares every time i go to sleep? they are to the point where i feel mentally disturbed and unable to function when i wake up.

Dreams are windows: to your unconscious mind. Nightmares can represent problems which your unconscious mind is trying to figure out and solve. (Unless they are side effects of medications or drugs which should be investigated as well). Doing a psychological consult to figure out what the dreams are trying to alert you to would be beneficial. In psychotherapy dream work is very interesting and helpful.
