In general, we don't: We doctors don't do that. The machine does that. The machine is the system that causes all these undesirable effects in delivering healthcare. There is enormous demand for healthcare (many people needing help), but a limited supply of doctors plus an unwillingness of society to move more funds into training and hiring more doctors, to better match the demand (actually, more complicated than that).
