See below.: Every doc is different, but yes. Please write out your story. Try organizing it by: medical issues / hospitalizations surgeries family history social history - alcohol, tobacco, etc. Medication list immunizations screening tests (colon, breast and cervical).

Family practice: Coming to a new dr is sometimes scary. The first visit is to find out if you two are compatable. If your health history is too complexed he may feel it would be better to see a specialist bringing your medical history and med list would be very helpful.
