Usually not: Second shot of hepatitis B vaccine should not cause painful lymph node enlargement. If you have painful lymph nodes, it would be prudent to consult your doctor.

Yes: Yes, the Hep B vaccine can cause a painful lymph node on the side the injection was administered. You can use Tylenol (acetaminophen) or Ibuprofen to help with the pain and swelling. If the lymph node does not get better within 7-10 days, or if it worsens, do see your Primary Care Provider. Hope this helps. -Dr Smith
