Fissure: Typically a rectal fissure is the source of severe rectal pain and bleeding. The hemorrhoids are less painful, cause bleeding, and often hide the fissure located under or deep to the hemorrhoid. Initial treatment is softer, smaller caliber stool with bowel regimen.
