Monthly hormones: it seems to me that the one thing you didn't mention is when is this the most sensitive. In most young ladies, breast pain will follow the ups and downs of the menstrual cycle. Fibrocystic breasts are the most sensitive. As long as all those studies are negative, this would be the most likely cause.
