Different causes: Menses occurs when there is not a pregnancy, so that the body can start over to get pregnant. Early pregnancy bleeding occurs for mostly unknown reasons. About 1/3 of women have early pg bleeding, perhaps 50% miscarry (1 reason) and then it stops with a normal pregnancy to follow. An ultrasound can reassure you of normal development at about 6 wks. Fetal hr means loss only 3-5%. Best wishes.
