X ray: Back pain from cancer is due to metastatic disease. if there are no symptoms of malignancy then rare to have spread. Common lesions are lung in the male as well as prostate in the older male. Other lesions usually advanced with the primary causing symptoms. When questioned by a physician dx established by showing lytic lesions expanding vertebrae as seen on MRI.
