It means: you probably had a previous inflammatory/granulomatous process that caused it. For example, histoplasmosis, TB, sarcoid. Occasionally treated lymphoma, certain metastases, and certain dust exposures can do it. It is possible that the cause is not clinically relevant at this point, but speak with your doc about what this finding may mean in your case.
