Has anybody died off of using the drug zoloft (sertraline)? and if so what was the major reason why?

Zoloft (sertraline): Scattered cases reported with any antidepressant, with main causes being sudden death from heart issues, to suicides to unknown cause. I routinely order essential lab workup, monitor heart status, & assess for suicidality prior to starting antidepressants.
