Either: A good way to differentiate will be to try running or going on a treadmill without using your upper body strength. If that reproduces the pain, cardiac causes will have to be ruled in or out. If it is that only upper body movements such as lifting, pushing, etc are causing the pain chances are that the pain is musculoskeletal. Do mention your symptoms at your next well visit.
