Increased fall risk: sounds like you have a lot of medical issues. I highly recommend that your discharge instructions include home healthcare (if you do not have someone to help you at home). They will be able to help you with getting your medications in order & getting your environment safe enough to avoid falls and injury. Keep all your follow up appointment with your primary care physician and any specialists
