Diabetes: It is very common to have swollen ankles as a diabetic. Hypertension (high blood pressure) often comes along with diabetes, and the veins can become overloaded with fluid. If the valves in the veins are having trouble pushing blood up the leg and back to the heart, swollen ankles are the result.

Venous disease: If your friend has type ii diabetes, one way to think about this disease is "dis-obesity." obesity then increases the pressure on lower legs veins, both superficial and deep. A venous reflux study may point to this diagnosis as venous insufficiency causes legs to swell. Diabetes itself doesn't cause the swelling, the obesity increase this venous reflux risk.
