Depends: Extrusion (orthodontics) will pull up the bone and gingival tissue with the extruding tooth, whereas periodontal surgery will probably leave some soft tissue cosmetic defect as it is removing gum and bone. But as the tooth is extruded, the portion at the gum level is narrower in diameter than normal, possibly leaving larger spaces between the teeth. I recommend getting a periodontal consult.

Depends. Implant?: Depends on the tooth. Get a couple of opinions. Considering the cost, and the expected life span of such a tooth, another option to explore is extraction, implant and crown on implant. Probably a better long term solution.

Conservative: The most conservative approach is rapid orthodontic extrusion, which leaves bone/gum behind, eliminating need for surgery. Slow extrusion brings gum/bone along with tooth movement, necessitating surgery. Crown lengthening is a surgical procedure. Also Root canal/crown more conservative than implant/crown.
