Spine not kidneys: Persistent pain or tenderness in the back near the kidneys is often due to underlying spinal issues, such as degeneration of the lumbar discs or arthritis of the spine. Pain from the kidneys themselves is usually intermittent and quite severe, whereas spinal pain tends to be more chronic and achy.
