Aortic aneurysm: This patient does not have symptoms when he eats but the symptoms seem to occur randomly. If it was a stomach ailment, one would expect him to have symptoms with eating. Basically, this pain sounds more vascular than a problem with his gastric system. He should be worked up with a contrast based ct scan.
