Ear infection?: It could be wax or it could be infected discharge from his ear. When an ear is infected the ear drum becomes red and inflamed, generates heat which can cause the wax nearby to melt. Usually if the ear drum ruptures due to an ear infection the child has severe pain right before the perforation occurs. Then the child has relief. You need to bring him to his pediatrician for an evaluation.
