Tough call: If related to the esophagus: could be mechanical stricture or narrowing (possibly from severe reflex, congenital issue, tumor, immune disease like eosinophili esophagitis), or uncoordinated contractions of the muscles as food goes down. If the problem is in the mouth, could be an issue with saliva productions. Virtual consult may help with guidance, but exam and EGD likely necessary.
