Not always: X-linked dominant diseases are distinctly rare. One might notice that dads never pass the disease to their sons and always pass it to their daughters, and in a large enough pedigree the pattern might become clear. Also remember that new mutations occur, non-paternity can occur, people may not know they're adopted, and cases can be missed.
