Endometriosis: People with heavy periods frequently have pain to varying degrees Two relatively common causes are 1) lining tissue of the uterus is outside the uterus as well and its bleeding causes pain, 2) a substance Prostaglandin G causes the uterus to spasm causing pain. Using 2 Aleve (naproxen) gels 2Xs a day until heavy flow stops helps. Birth control pills help. See your gynecologist soon as a Dx would help.
