Impossible to say: First of all, a significant disc herniation may not improve with conservative therapy. If it does respond, it depends on the severity of the nerve compression. Give it 1-3 months.

Variable: A herniated ("ruptured") disk can respond nicely to physical therapy and the body's natural healing processes. In my experience it can take weeks to months. But in some cases, perhaps 10-15 percent, surgery is needed. Indications for surgery include significant weakness, numbness, persistent pain, pressure on the spinal cord, trouble walking, or failure of conservative management.
