Confusing question: Tolerance to a medicine after taking it for several years is unusual in my practice. Tolerance, if it develops, usually happens within weeks, and I raise the dose of the medication to its full therapeutic level. In my patients, the therapeutic dose remains effective for decades. If that's not your experience, please see you treating physician. There may be something interfering with your meds.

Tolerance?: Tolerance isn't a given. You might simply not be at the dose you need. Did you switch from brand-name to generic, or from one generic to another? Their bioavailabilities may differ. It's known that one generic was simply not bioequivalent to Concerta, so patients had to go back to brand. Also, make sure you don't have obstructive sleep apnea. OSA interferes with benefits to ADD from Concerta.
