Many things: Swelling of the ankle can be caused by inflammation due to many things. Trauma can cause damage to ligaments, tendons and bone resulting in swelling. Gout, pseudogout or infections can cause swelling. Decreased blood flow (for a variety of reasons) from the area may also mimic this. Evaluation for these in the clinic would help delineate the cause.

One side or both: swelling in one ankle only mean the problem is in that extremity from vein problem superficial or deep,to a joint problem from arthritis to sprain to soft tissue and ligament problem bilateral mean a systemic problem heart or kidney is it only swelling or there is pain associated with it answering some of these questions might help to a diagnosis and that will create a treatment plan
