Complex: Polycystic ovarian syndrome is complex. It involves an imbalance of the female hormones as well as increase in male type hormones. You do not necessarily have to have any cysts on your ovaries to have PCOS. Thirdly it involves insulin resistance and that is why you were placed on metformin. Researchers are still looking to understand the cause(s) of PCOS.
