What would happen if doctors did not clamp off a belly button after giving birth? what would it look like?

No different: when a child is born, we clamp the umbilical cord not the bellybutton. People with "innie" bellybuttons do not have anything different done with the umbilical cord at birth then people with "outie" bellybuttons. People with "outie" bellybuttons had a small hernia at the side of the belly button when they were a child. This causes the "outie" appearance. I hope this answers your question.
