What could be wrong if a fracture hasn't healed at all after a month?

Fracture healing: In general, there are 3 stages of fracture healing. Stage 1-Inflammatory Phase with hematoma formation. Stage 2-Repair Phase with bony callus formation. Stage 3-Remodeling Phase with consolidation of callus and return of relatively normal bone structure. At 3-4 weeks from the initial injury there will be minimal or no evident callus formation on xrays. The first real signs are seen at 6 weeks.
