Tight shoes: Little toes get quite a bit of trauma from shoes, especially in fashion-conscious women. High heels with narrow toes cause the foot to jam forward into the toe box. This, in turn, can loosen the toenails, causing them to "autoavulse". The little toe is often the first affected. Keep the toe clean and watch for infection. The nail will grow back.

A few things: If you injured the toe, you may have injured the nail bed and root from where it grows, and that can cause the nail to loosen and fall off. Infection of the nail and supportive structures can affect the integrity of the nail, causing it to fall off. Usually the nail will grow out over a period of months.
